  • Our membership is a monthly paid program. Get one month FREE when you purchase a full year in advance. Memberships paid in full are non-refundable for the year.

  • The monthly membership fee will be auto drafted from the credit card on file on the 1st of each month.

  • Cancelable at any time, before the 20th of each month, after 3 months of membership. Simply email us your request.

  • Unused membership treatment can be rolled over up to 30 days and then will be forfeited after the allowed 30 days.

  • You must schedule any services in advance to ensure you receive the date and time you prefer. We are not responsible for last minute appointment requests that cannot be accommodated, but we will do our best.

  • Your perks are non-transferable and cannot be used in conjunction with any other promotions or offers.

  • Monthly membership fee can’t be combined with any other offer.

  • No cash value, refunds or credit for unused months

  • Revived Aesthetics reserve the right to modify or cancel the Membership program at any time for any reason

Your $25.00 Referral Bonus will automatically go into your client account for use on your next visit.
